Purpose of the questionnaire
Top management is accountable for the organization's internal control.
Managers and other staff have valuable information,
whether internal control works as it should.
Some pieces of this information can be collected in a survey, using a questionnaire.
The responses are analyzed and conclusions are made.
Conclusions can be reported e.g. in a statement that
top management gives about the organization's internal control.

Parts of the questionnaire
A questionnaire should carry information at least about the following.

Introduction of survey: why the survey is made, based on what,
to whom is it addressed, how the responses will be processed and by whom,
and where and when the results will be reported?

Replying: How to fill in the questionnaire,
when and how to send the responses,
are the responses given in anonymity,
and who to contact if more information is needed.

Text for questionnaire
This survey is made for finding out staff’s opinions and findings
about internal control in our organization.
Results of the survey serve as one source of information
when top management makes its statement about internal control
as required in [name of the regulation].

Internal control is an organization's procedures and arrangements
design to keep performance or a state of affairs within
what is expected, allowed or accepted.
These procedures and arrangement include Control environment, Controls,
Accounting systems, and Determination of Risks.
They are explained later.

The survey is made by the Internal audit department
at request of the director general.
Respondent information is kept secret in all phases of the survey,
including analysis of results.
Results of the survey will be reported to director general by [date]
and they will be used only for the statement mentioned above.
